(BES Students can call 826-3624 for screening appointment @ BES)
WHO:
· Kindergarten - 5 years old by August 1st
· Preschool - 4 years old by August 1st
· Preschool - 3 or 4 years old by August 1st with Disabilities Including Speech

RECORDS THE SCHOOL WILL NEED:
· Birth Certificate
· Shot Record
· Proof of Income (preschool only)
· Proof of Residence (new households only)
· Health Physical / Vision Exam / Dental Screening
(School health forms are available at schools and will be handed out at the event.)
